During the 2025 Australian Association of Gerontology Conference, held in Alice Springs this November, frailty was one of the main topics of discussion. With age comes frailty; however, it goes unnoticed, and during screening processes for this, inconsistencies arise. The ACH Group took on a proactive approach to frailty and came up with frailty tools. 

With the Support at Home Program in place, research has shown that this is one of the best ways to reduce hospitalisations. During the AAG conference, different in-home care models were compared, including transitional care, integrated care, hospital in the home, short-term nursing interventions, home-based primary care, and community nursing.

The research shows that in-home care models not only cut back on hospitalisations, but they also reduce emergency department visits and shorten the length of stay of older people in hospitals.
